Thank you for your interest in becoming a customer of HollyFrontier Refining & Marketing LLC.
There are some standard forms we need completed in order to set up your account.
We conduct a detailed review to determine whether open credit is warranted.
Please provide us with the following information:
- Credit Application
- Bank & Trade Reference Form
- Electronic Funds Transfer (EFT) Authorization Agreement - We require all payments to be made by automatic draft.
- W-9 - Request for Taxpayer Identification Number and Certification
- Last 2 years of audited company Financial Statements (Balance Sheet, Income Statement, Cash Flow Statement, and Notes)
- Personal Guaranty Form
- Current Personal Financial Statement
- Tax Information
- Authorization to Release Credit Information
- Credit Package Cover Letter
